Brief Answer:
Insulin should be started at the earliest

Detailed Answer:
Hello,
I can understand your situation.

The increase in blood sugar by corticosteroids is very high and usually, we start insulin therapy to get it to normal levels.

Oral antidiabetics may lower it but is too high to get back to normal levels with them.

So, I would suggest to go te ER and start insulin therapy at the earliest.

If it is impossible, please take the maximum dose Glipizide (40 mg/day) and 2000mg of Metformin (if the renal and liver function is normal).

Of course, it is too high to be treated at home.

Hope the information will help and please seek immediate medical attention.
Kind regards,
Dr.Mirjeta
