Question: I have been on Lo Loestrin Fe for a couple of years now. How can I manipulate my menstrual cycle next month? I am taking an important trip and would love to not worry about that.

I am Dr Ramadevi Wani. I will be answering your concerns today.

This month take only the blue tablets for 24 days. Do not use the 2 estrogen ( white tablets) and 2 Fe pills ( brown tablets). Throw them away. Instead start with the first active pills ( blue pills) of the new pack immediately (on day 25). By doing so you can postpone your menstrual period by 4 weeks.

Thank you for your response. Typically, I start during the 3rd week of pills. I on on day 16 today. Usually, by day 18 or 19, I have started. I have always been like this since I have been taking these. Do you have any different suggestions based on that?

Welcome back.

In most women bleeding usually starts on 25th day. In your case bleeding is starting when you are still on active pills. In such a scenario starting a new pack on 25th day is not going to help.

I suggest that you shift from today to contraceptive pills containing higher estrogen like Cryselle, Lo-Ogestrel 28, Levora, Nordette etc. The high estrogen content in these pills will help. However there is a very slim risk of breakthrough bleeding when you shift to new formulations. But this is the best option.

If you can tell me on which dates you do not wish to have bleeding, may be I can think of other options.

Thank you for your response. I am actually trying to plan ahead for next month. I don't need to be bleeding on August 11-14. Normally, that should be the week I start for next month's pills.

Welcome back.
This is a tricky situation.
As I mentioned before one option is to start contraceptive pills containing higher estrogen levels. Taking them continuously (avoiding the inert pills and starting the new pack immediately after the last active pill of previous pack) until 15th August.

The other option is to start progesterone (Primolut-N, Provera) 10mg daily once you finish the period bleeding this month and continue till 30 July. You should expect period within a week of stopping the pill.
