Brief Answer:
Injectable B12 has better outcome

Detailed Answer:
Hello Dr XXXXXXX

Injectable vit B 12 are superior for achieving normal sr B12 level as intestinal absorption as with oral medication is not there, so I will advise to take intramuscular injection alternate day at least 10 ,and then repeat blood test.
Vit B12 helps in nerve conduction so normal level will improve problem of forgetfulness and tiredness.

For vit D you can take cap Gen D3 60 k once in a week for 3 months.

If you are suspecting malabsorption syndrome then you can do test for Intrinsic factor, folic acid, TIBC. Homocystine level will come to normal once you take B12.

Smoking will affect your absorption process so better to stop it.
