Brief Answer:
Could be due to depression

Detailed Answer:
Hello thanks for asking

I have read your report in detail and come across following important points-

You are a 34 year old male with freelancer pvt. job, with good education and poor marital life. You have habit of not paying attention to important events of life like job interviews, tests or meeting someone. You have withdrawn behaviour, decreased social interaction since last 2 years. You have separated from your wife about 2 years back. Laziness, inferiority complex and loss of confidence are other complaints. There is no significant abuse history and past history of mental illness. You are not taking any significant medicines except inhaler for asthma.
The doctor has prescribed Modafinil and Elcafort for these problems.

Now coming to discussion-

The psychological report itself is mentioning many important symptoms like decreased social interaction, withdrawn behaviour, poor confidence, inferiority complex, skipping important tasks, tiredness and laziness etc. As per my opinion these symptoms are part of mild to moderate depression. It is not common to see depression since childhood but your symptoms has aggravated after your divorce.

The symptoms like missing important events or avoiding till end can be considered normal in childhood but the symptoms that have developed after your separation can't be overlooked. Separation is one of important life event and can precipitate depression in susceptible individuals. As per my opinion there is need of addition of some anti-depressant medicine also.

The drug Elcafort is a multivitamins supplement. It is used in a number of neurological conditions.

Modafinil is a drug used to improve day time drowsiness and it promotes wakefulness. This drug is also used to improve sleepiness and tiredness seen in depression.

So please consult your psychiatrist again for antidepressant drugs. Medicines of SSRI class especially Fluoxetine, Escitalopram can be used in your condition. These drugs improve depressive symptoms and will help in day time tiredness also.

Apart from this medicine like Amisulpiride can be added to your prescription. The drug in low doses prove very useful in negative symptoms of depression. You have many negative symptoms like lack of confidence, inferiority complex etc. The drug will improve mood and these symptoms.

Non-medicinal treatment especially Cognitive Therapy can also prove useful in your case. The therapy help to explore faulty cognitions that develop since childhood and therapist and patient work together to correct them. Consult a good Psychologist for that.

Practice yoga daily and exercise daily for at least half hour. Don't feel inferior you will improve in some time.

Hope this helps you, you can ask again if you have more doubts.

Thanks

Brief Answer:
Antidepressants

Detailed Answer:
Hello again

Sure, first of all use of other drugs-

Censpram is Escitalopram. This is a anti-depressant drug and help to improve your low mood and poor drive. I guess you are taking the drug in morning hours.

Zapiz is clonazepam. At 0.5 mg dose it has anti-anxiety action. But it can cause improvement in sleep and in some individuals it results in severe sedation. The drug shouldn't be taken in day time as it will further aggravate your problem of sedation in day time. Don't use this drug for long time, it can make you dependent.

Lopez is lorazepam. It is also a benzodiazepine, help to increase sleep and anti-anxiety drug. If you are taking the drug in evening hours its good and don't take medicine in evening.

You have problem of day time sedation. One reason for this sedation is benzodiazepines dose. It is possible that you are having excessive sedation in day time because of heavy use of two benzodiazepines drug (Lopez, Clonazepam).

You just have to cope with situations and continue to take Censpram, Modafinil and you will see improvement.

You can ask again if you have more uncleared doubts.
Thanks
