After carefully going through your query, the chances of compression fracture are negligible as these occur while sudden, violent vertical forces are applied on spine that is from head to toe as in fall from height or jerky rides. While in open heart surgery you will be in supine position that is on your back with gentle and slow distraction by rib spreader.
As far as osteoporosis is concern I usually give calcitonin nasal spray. You may have it prior to surgery after consultation with your doctor. Take balanced diet rich in calcium (1000mg/day) and vitamin D (1000 i.u. /day) along with vitaminB12, B6.
