Brief Answer:
Rarest possibility can not be ruled out

Detailed Answer:
Hi,

Welcome once again to HCM.

Thanks for posting your query.

I would have come across around 6000 HIV positive people so far in my career since 1989 onwards, among which I met two cases who had HIV becuase of incorrect use of condom and subsequent tear.

But it is very rare possibility. At the sametime I had come across around hundred individuals with the history of condom breakage and remain free from HIV.

Any queries in this regard is most welcome.

if you were to put a percetage on what group has the bigest problems with hiv
XXXXXXX men - %
straight men - %

Answered by Dr. Sankaranantham Murugan (22 minutes later)
Brief Answer: XXXXXXX men are more at risk.

Detailed Answer:
Hi,
Welcome back.

Of course the risk is more with XXXXXXX men than with the men having normal vaginal sex beyond any doubt.
The reasons are:
1. Vagina is designed for sex with extendability and lubrication whereas anal canal is not. So it is liable for injury and subsequently for infection due to exchange of body fluids.
2. XXXXXXX men would likely be to have multiple partners compared to hetrosexual men.

You say you seen about 6000 cases and out of which two were caused possibly by condom break.

I am interested in the others , how did the rest
Get infected with HIV ?

Where most your cases XXXXXXX men ? People shooting up drugs ?

Where these people given HIV by their mothers ?

I'm trying to get familiar with this disease .
So I am better informed . I get tested regularly and I'm HIV negative , but I get my scares at times.

So far I hear that the threat level of this disease
Seems quite low , even in my lifestyle which I have had sex with over 100 prostitutes in the past years . ( I do not see street walkers )

Please give me more detailed view on this subject from your perspective .

Thank you

Brief Answer:
85% of people acquire HIV through sexual route.

Detailed Answer:
Hi,
Welcome to HCM back.
85% Acquired HIV through unprotected hetrosexual contact or through men sex with men. Very few through blood transfusion and few through mother to child transmission and rarely through intravenous drug abuse. This may have slight variation to country to country and region to region. There are 30-40% of the spouses of the infected individuals remain free from HIV in spite of their regular sex contact for years together. So some people can escape from infection even though they are at risk.

85 % is quite a number of 6000 patients .
Roughly of this number how many would you estimate were
man to man transmission ?
doctor
Answered by Dr. Sankaranantham Murugan (4 hours later)
Brief Answer:
Quarter to 1/3 of cases would have acquired HIV through anal sex.

Detailed Answer:
Hi,
Welcome to HCM.
I don't really have the incident exactly. It would be around 25% of the sexual transmission cases in this part of world.

Reading online from many doctors
HIV seems like it is very difficult to get from one
Time unprotected encounter even if a person is
HIV positive (female )
Approximate statistic is 1 in 2000
These chances increase drastically if you continue
Having sex with this person for longer period of time .

Would you agree with this statement ?

Also they say condoms protect you if used properly 80 % of the time .
I'm confused because if the condom is on
And does not rip during sex to me it seems like
It's 100% success . Please try to clarify this for me .

Thank you doctor

Brief Answer:
All exposures are not necessarily infectious. Condoms offer good protection

Detailed Answer:
Hi
Welcome back
There above statement is absolutely true. All the exposures are not necessarily infectious. Nearly 30-40% of the spouses of the infected individuals remain HIV free in spite of their regular and consistent sex contact with their infected spouses. They are called as sero-discordant couple. So you can relax.

I will ask you last question ,
For probably about a month or month and a half
I have been having some minor pain in my armpit
(Left) I am scared that it may be related to ARS

Also , I have been sick for last 1-2 weeks but so has my whole family including the children .
I have not had any fever .

I really don't think I have any other issues
Just 2: left armpit pain and recovering from more then likely a flue .

Oh , also at night I noticed my arms tend to
Fall asleep . I don't know if that is related
What could this be ?
ARS ?

Answered by Dr. Sankaranantham Murugan (2 hours later)
Brief Answer:
HIV testing only is the way to rule out or confirm HIV

Detailed Answer:
Hi,
Welcome back
Sickness with all family members itself ruled out the possibility of HIV infection. Pain over armpit not related to HIV or ARS.

See , two months ago I had a condom break
On me during vaginal sexwith a prostitute.
Since then I have had sex with another 5-8 women. Always protected sex.

But this one time that condom broke is a concern.
About 8 days later I took HIV test for early detection . It came back negative .

But I'm concern due to the armpit pain I've been having . Plus how reliable are the results after 8 days ? It was I think called hnr test .

Brief Answer:
HIV PCR becomes confirmatory by 4 weeks for HIV 1 infection

Detailed Answer:
Hi,
Welcome back
HIV RNA PCR tests are two types. They are Quantitative and Qualitative. Among RNA PCR Quantitative is available widely compared to qualitative. Qualitative RNA PCR is useful for diagnosis and Quantitative is useful in assessing the prognosis of the disease. HIV PCR will detect the disease as early as one week onwards, but more conclusive after 3-4 weeks. So your test cannot be taken as a conclusive one. Moreover Most of the PCR are able to diagnose only the HIV 1 infection (99% common), not the HIV 2 infection (less than 1 %).
Your symptoms are not related to HIV.
