What Are The Chances Of HIV Infection After Protected Sex?

Question: Hi I am a 28yr old unmarried guy. I have had protected vaginal sex with 3 call girls in 2 weeks(recent encounter being day before yesterday)I totally regret for what i did. Though I was careful in terms of maintaining condom all the time. I am afraid of HIV due to oral sex(Fellatio). I have done HIV antibody test today and the results are negative. I am afraid that there are few symptoms which are matchin with the HIV symptoms i have found on net. I have cold, slight fever, my throat feels itchy, very slight pain during urination, slight pimples on my back and a tiny bump on my head which pains when touched. All these symptoms i got after the exposure. Please guide me wat to do?? I am very much frightened.

Welcome to the forum and thanks for posting your query to the forum.

I can understand your deep concern for the possibility of HIV after the various protected intercourse in last 2 weeks but with oral sex.

Regarding above condition, it is a good news for you that academically the risk of transmission of HIV is almost negligible and in my clinical experience it is nil . There are certain medical reasons for the same:

- Your intercourse in all the 3 episodes were protected. In protected intercourse it is practically impossible to get an HIV infection. So relax from the part of protected intercourse.

- For the HIV infection after oral sex(Fellatio)- it is not seen practically that there can be any chance to get the infection by oral sex(Fellatio). Medically HIV virus can not spread by oral sex in either case from male to female or female to male. There are least of chances only when both the partners have blood discharge from a cut wound that get mixed in adequate amount. In mouth there are certain enzymes which makes the HIV virus inactivated.

An important thing is that you don't know the status of any of female. But even if we consider that if any one of three have had HIV infection then also there are no chance of HIV virus to get spread from mouth of female to your penis. It is because as soon as the virus comes in salivary secretion, the enzymes in saliva stripes of the virus and makes it inactive and non infective.

So relax for the part of oral sex(Fellatio) and getting HIV infection because of simple medical reason given above. In my clinical experience also I have never seen any case of HIV infection after oral sex(Fellatio).

The antibody test is though valid after minimum 6 weeks but even if you want to confirm your negative status then RNA PCR can be done at this time.

For the part of the symptoms you are feeling are just due to anxiety and nothing else. In HIV there is high grade fever with severe flu like symptoms. Just check your temperature it must be below 100. In HIV the temperature is never below 101 Fahrenheit. The rest of the symptoms are also non specific, so allay all your fears and relax.

RNA PCR is a blood test and can be done any time after 2 weeks last exposure. It detects directly the HIV virus and therefore is the most reliable and useful test in your kind of situation. It can be done at any reputed lab without any requirement of prescription.

As your last exposure was 2 days back so wait for another 12 days.

In opinion and as per best of my clinical experience I am quite confident that your test for RNA PCR would be negative.

Dear Sir,

I write this as i am highly terrified. In the past one week, the symptoms include, fever during nights, small pimple like bumps on my back, a canker sore in the mouth(attached the pic for your reference), fatigueness continues and has increase over few days. And more terrifyingly I have small pimples on the skin under my penis. I am extremely worried, though i was satisfied by ur answer, there are still 7-10 days pending to undergo RNA PCR test, please let me know any medication to avoid HIV. Though i was worried about HIV spread due to oral sex but in few days i have started worring about condom break(I have not cheked, so i am not sure). Please guide me further.

I completely understand your concern and fear. For the recent symptoms experienced are not related to HIV as per my clinical experience.

The picture of the canker sore has also got no relation with HIV and same is true for rest of the symptoms too. The picture shows a simple aphthous ulcer which is most commonly seen after stress.

I reassure you again that these symptoms are quite simple and can be seen in any day to day illness. So do not correlate these with HIV.

There can be multiple possibilities for pimples type of thing so do not become anxious for the same.

You can send few pictures of the pimple kind of thing on penis as well back and I will guide you for the same accordingly.

Note down you fever temperature and tell me the exact Fahrenheit temperature of fever. In HIV it is always beyond 102 Fahrenheit.

So you let me now the temperature and can send pictures of the penile bumps and bumps on the back.

I have gone for HIV 1 DNA test as the RNA PCR was not available. And the results says negative. Please let me know whether DNA test suffices the detection of HIV after 2 weeks of exposure or shall I go for RNA test in the nearby city. Eagerly waiting for your answer sir.

First of all let me give my blessings and good wishes for negative DNA PCR result. As it is two weeks since exposure so is quite sufficient time to declare your negative status by DNA PCR test.

DNA PCR can detect the presence of HIV virus as early as 10 to 14 days of period. In my experience those persons with DNA PCR negative at 14 days are totally clear form HIV.

Though some of the times RNA PCR is considered to be better than RNA PCR but academically speaking as well as in my own clinical knowledge and experience you are clear form HIV and therefore do not require any further testing.

Dear Sir,

Further to add in my case...I have uploaded the pics of my skin condition on my back. this skin condition(small pimples all over) i have noticed in the past 2 weeks and i am a little worried. My DNA PCR Test turned HIV negative in the month of november. Please let me know is my skin condition a worrisome for HIV and also please let me know if any other followup tests i would have to go for to be 100% sure of my health condition.

Dear Sir,

I am very much tensed,though i have always got convinced by your response something new comes up which is making my life very tensed. I have attached the pics of my penis(sorry if its rude to post) u can see small spots on the same it was not there before. also on one of my testis i noticed a small cut which was bleeding also two days back and now it is gone. I dont know what is happening to me i am very very worried right now. I am planning to go for HIV1 RNA PCR quantitative testing in another 15 days. I have developed light cough also these days, i am so tensed. Sir, please tell me is it HIV or Herpes or any other type of STD... if u have any doubt please tell me which test i have to undergo i will definitedly undergo the test and post here. Is there any test that tells HIV & Herpes at the same time...U have known all my symptoms please tell me what is the disease that is troubling me.

1) Initially I had light fever one month back
2) I had mouth ulcers even i have posted the pic also here
3) I feel extremely asleep, in fact i sometimes worry while driving my bike i may sleep...uncontrollable sleep
4) I had small spots on my back i have attached pics also last time
5) Now these small spots on my penis ..it is really worrisome
6) I have developed cough also these days.

Please help me sir.

I can understand your concern for the present problems. I have gone through the latest pictures of the penis and can make out that this is quite normal natural appearance and there is nothing abnormal in the same.

You should know the fact that our penile skin is the thinnest skin on the body. It is such a thin skin that even the blood vessels are directly visible over it. This redness of the blood vessels is many times confused with some kind of skin problems such as red rash or eruptions or related thing. In your case also these spots are due to this simple normal phenomenon of the human penile skin.

All your other symptoms are not at all related to any sexually transmitted infection and are basically due to anxiety related to possibility of any STD. So relieve all your tensions and can go for simple RNA PCR for HIV and IgM with Ig G for herpes to get relived of the tensions.

Sir I am worried about the small white spots on the penis which are visible on the head of the penis. they appear like infection.

To tell you medically - Herpes lesions manifest itself as small vesicles (fluid filled blisters) which remains for just a day or two and then forms ulcers which are quite painful. These ulcers then heals in next 5-7 days.

To begin with also these vesicles are quite painful and itchy with discomfort and once the vesicles burst in a day or two then the ulcers which are formed are quite painful for next 4-5 days. So the symptoms can never go unnoticed.

In general herpes occurs by 7-14 days of period. In your case it has almost 55 days since the incidence so it is unlikely to get herpes.

You can see that there are no such above kind of lesions in yourself. Even then if you want to go for testing then it is the right time to go for the tests.There are two antibodies for the detection of herpes: one is IgM and the other is IgG. IgM starts appearing by 10 th day and disappears by 24 to 28 days. On the other side IgG may appear by 28 days to 42 days of the period. So clinically when we test for these antibodies then analyze the condition accordingly at this time.

The detection of IgM and IgG for herpes is done by blood test and reports can be obtained on the same day.

For your patch on the penis : I have already reassured about the penis issues and guided you that there is nothing abnormal in the same. Even then if you are worried about latest patch then you can again send a latest picture of the area so that I can guide you accordingly.
