Question: Hi
This is to inform you that i received a blowjob from a girl (sexworker) whose HIV status is not known to me. After this i consultwd my doctor. He sugested me a medecine Duovir(PEP) . I started using this medicine after 6 hours of this oral mouth to penis exposure. My question is that how much i am at risk of receiving HIV? And please suggest should i continue my doses for PEP..??

I can understand your concern for the risk of HIV after blow job. After thoroughly analyzing your situation let me first and foremost tell you that there is negligible risk of HIV by your kind of exposure.

To tell you simply: - there are two conditions in your case- either the girl is HIV negative where you have nothing to be told.

In other case let us assume that the girl's status is positive. In this case also there are least of the viruses present in the oral secretion. The saliva contains certain inhibitory enzymes which actually inhibits the viruses. So even if a person is HIV positive then also the viruses are inactive in the salivary secretion.

In my experience I have never seen any case of HIV positivity after blow job. So relax about any chances of HIV in your case.

For the part of PEP-you have not mentioned since how long you are taking the same. Kindly do let me know since how long you are taking PEP.

Thank you doctor for your answer. It really did help to reduce my tension a lot. I am quite relived. But as per PEP i am taking it for 2 and half day. But I have read about its side effects. Although my doctor has suggested only duovir prepared by cipla. But i think due to this medication i feel a flu like symtom for about twice or thrice a day and that too also for may be 10 to 15 mins. May be the fevervlike feeling could be result of my tension. I dont know. And i wont say that i have a stomach upset but went to potty twice. It was normal both the times. So what are your suggestion should i perform one liver function test after a week and lets see what happens. Or can i stop the medication? And should i take any HIV test? Please suggest .Thanks in advance.

Hello young fellow,

Welcome again to the forum.

In my view once you have started the medication then should take the complete course. The mild to moderate flu like symptoms are not unusual and will not going to cause any harm. In my patients either I don't start or u at I start then make it sure that the medication us been taken completely.

Regarding the testin- there is no test which can be performed at this point of time. At the earliest one can get tested after 14 days by RNA PCR. In my view all your tests would be negative at any juncture so just relax yourself.

Hi doctor.
Thank you so much for your suggestions. I would like to ask you that is it due to the medication (PEP) that I sometime have a vomiting feeling. I am taking it for 6 days and this vomiting like feeling is felt by me once or twice.
Please provide suggestion. And one more question do you advice me to take the hiv test or is it ok not to take the test. But i will complete my full PEP course.

Hello young fellow,

Welcome again to the forum. I can get from your query that you have the feeling if vomiting after starting PEP.

It is quite usual to get such kind if feeling after taking PEP. It is quite usual to have vomiting sort of feeling or bit nauseating after taking these medications.

In my patients I suggest them to take an omeprazole capsule for twice daily empty stomach. If still the feeling is not reduced then can ask your treating doctor to see the liver profile.

Take sample feeds at regular intervals and do not take any heavy meals.
