What Are The Chances Of HIV Transmission Through Protected Intercourse?

Question: hi doctor,

I had protected sex with CSW in germany, it was one heck of mistake and i will repent this for rest of my life.
due to stress i conducted ECLIA Test in Germany on 26th and 40th Day which came back negative.

After coming back to XXXXXXX i conducted test again HIV Antibody1/2 on 60th day, VDRL and HBsAG which came non reactive.

on 65th day i conducted CHLAMYDIA TRACOMATIS AND NEISSERIA GONORRHOEAE, PCR QUALITATIVE which again came back negative.

on 90th Day i went for HIV1/2 ANTIBODIES SCREENING TEST (HIV 1/2 & P24 Combo test (CMIA) at lal path labs XXXXXXX Index value as 0.07.

I consulted with one dermatologist and venerelogist specialist here, he said nothing will come till 6 months in report if you are infected. come after 6 months
which made me even stressed out, i have confimed myself from last 3 months and social activities have reduced drastically.

Feeling scared to death even to touch my family members. I am married and are planning to start family but i am scared to death getting
all weird thoughts which i cant type here.

Are these tests conclusive and confirmatory for HIV at 90 days, And other tests such as VDRL,Chlamydia annd gonorrhoeae?

coming to symptoms.

white saliva, dry mouth, one/two mouth and gum ulcers, headache, extreme tiredness, pain in calf muscle, sometimes pain after urination and sudden pain in groin region.
also i am seeing pale yellow urine from last one month regularly.

sometimes i feel tenderness in armpit, I have pain in the left groin area from 3 days and uneasiness in lower abdomen.

Please guide me what should be my next steps and if i need some more tests and i need your help very badly.

I can understand your deep concern for various Sexually transmitted diseases including HIV after the incidence at Germany.

Going through your complete query and interpreting all the reports let me first reassure you that as per best of my clinical experience there is no need of any further testing and you are completely healthy in terms of any Sexually transmitted diseases.

I diligently interpreted and correlated your symptoms but none of your symptoms are suggestive of HIV or any STD.

So young fellow relax yourself. The very first important point is that it was protected intercourse. In my clinical practice I counsel and follow so many cases of your kind of protected intercourse. With best of my clinical experience I never found any case to be positive for especially HIV.

With these latest of the antibody tests the results are confirmatory after 3 months. There is no need of any further testing now a days.

The various symptoms which you are experiencing are just due to anxiety states only. None of your symptoms are related to any STDs.

I hope you must be relaxed to know the simple facts regarding your concern.

can i consider myself free from HIV with this combo test, CMIA done in XXXXXXX on 90th Day? I was really stressed when
one doctor said come after 6 months. it is difficult to bear this stress for 3 more months, its killing me daily.

Are the tests done chlymdia and gonnorhea PCR on 65th day confirmed and VDRL done on 60th day confirmed.

I am sorry if i am reapeating same questions but i am sure you can understand the situation i am in.

Can i resume my normal life with my wife i mean unprotected sex?
and shall i consult with physician keeping this encounter aside so that i am able to conclude treatment.

I need your expert advise and i will go with your advise,

With the best of my clinical and academic experience I reassure you fully that you don't have any Sexually transmitted infection.

I have already given you a valid medical explanation for HIV. After 3 months of incidence the combo test is confirmatory. There is no need to wait for 6 months in your kind if situation of protected intercourse.

For chlamydia and gonorrhea, PCR test is totally confirmatory at 65 days.

For syphilis also there is no need to go for any further testing.

So I reassure you again and guide you to relax for any issues.You can very well resume your normal sexual life with your wife.

You can meet a physician for your generalised symptoms keeping all your fears aside for any STDs.

Thank you doctor, like you said HIV, Syphilis, Chlymdia and Gonnorhea are not the cause and ruled out now.
Another test which i did on 60th day, both CMI Method

Hepatitis C Virus Antibodies - Non Reactive
Hepatitis B Virus Surface Antigen (HBsAg) - Non Reactve
Are these confirmatory or did any follow up tests required?

today i went to GP and did some test,did not mention abt this incidence.i have given abnormal values from reports below.
CBC test --> Neutrophil - 74% (Normal range 40-70, 1 month back it was 69.5%)
lever function test --> SGPT - 43 (Normal range 0 - 40)
GGT has increased from 22 IU/L to 48 IU/L in one month (normal range 7 - 50)
Urine --> Appearance slightly turbid, Pus Cells 10-12 (normal range 0-5/hpf)

My question, are abnormal number in lever function test has anything to do with Hepatititis infection?

Doctor did scanning and it showed grade 1 fatty lever, but timing of this is what concerns me.
Kidney scanning was normal but yellow urine is there and sometimes i have pain while urinating.
I have given urine for culture report and based on it next treatment will follow.
for now, i have been antibiotics for 5 days.
Please suggest.

Regarding Hepatitis part - the test result is confirmatory and no need to go for any further testing related to hepatitis of infective origin.

As you have fatty liver so you have increased liver function tests. The fatty liver part is not related to anything with sexual contact. These will come to normal as you recover from this part.

The blood counts are not very abnormal and are quite in normal range. A mild difference could be seen in everyone.

Drink plenty of liquids and reduce your stress. Liquids will flush your urinary system.

I reassure you that these things are not related to sexual contact.

thank you doctor. your words have given me some sense of relief, I will follow your advise and go ahead with normal life. i will follow treatment suggested by GP. thanks a lot doctor you are doing a great job. god has taught me a lesson hard way.. just hope everything comes out clear as you have already mentioned and I can live normal life happily. no one should do such a stupid mistake and risk lives of so many people around you. hello again doctor, the urine culture reports are normal but the symptoms are still there such lower abdominal uneasiness, sometimes tenderness in abdomen.
sudden pain in penis. Yesterday i found some white material at the tip of penis, GP said no need to worry. He also checked my groin area and said no issue there as such.

GP did say that Pus cells 10-12 are not normal in male and that made me more worried and stressed.

Doctor has given me antibiotics and some other tablets such as --> DOXCEF CV, PYRIDIUM and RABESEC.

I am taking these tablets but i have this continuous feeling that i am not well.

what could be the cause of high level of pus cells, in online forums i read its either STD or UTI, here in my case both have been ruled out.

My stress levels have again gone up and feeling really low. Please help me.

In my view pus cells could be present due to mild urinary tract infection or due to mild kidney infection. I suggest you to meet a urologist for the same. For the time being period your Gp has given a good treatment which you can follow as per his guidance.

The most important part of stress of STDs is now out so you should not divert your mind towards the same.

thanks doctor i am continuing the treatment suggested by GP. still i am finding it really difficult to control stress with anxiety and fear of contracting STDs. can you recommend some antidipressents. with any minor change in body my stress level is going up. i know you said that i am free from STDs but i am not able to control stress level and not able to take my mind of the thing.
