Question: i had a body to body message with unknow HIV lady ... there was no kissing no hand job no blow job and no penetration . where i first sprayed my penis with a spray for delaying ejaculation, and then wore a condom. I laid on my back and the lady started rubbing here vagina to my penis. until i think she finished. i moved her and i noticed that the cobdum was almost taken away. i replaced the condum and let her lay ob her face and i was only touching the vagina from behind, with no penetration. tehn i made her rurn where she was facing me then i rubbed my penis to her vagina and vulva . i ejaculated in the condum ....i moved and cleaned my penis immediately with water. Now Is the vagina secretion could lead to HIV transmittance? since when i was laying at my back there might be secretion on the penis ... I was a advised that PCR in 9 days will be conclusive for HIV ? Is there other tests i can to undertake to make sure that i did not get any infections from this incidence? Could some one have HIV with out penetration?

now it is 8 days and i am having flue symptoms..and no feeling for eating?
I made the attached test on the 8th day .do I need to make any other test

I have gone through your query and I have understood it

Since you noticed that the condom has slipped during sex and therefore was not entirely protected therefore it does pose a risk for HIV as well as other STD's. Genital to genital rubbing is a risk factor for transmission of HIV

HIV PCR RNA can detect viral RNA from 9-13 days after infection. At 9 days some of those who are infected may not be detected by this test i.e would be false negative.
It would be safe to assume that at 2 weeks almost all those who are infected will test positive by this test.
Therefore I suggest that you get yourself tested at 2 weeks with HIV PCR RNA for a definitive diagnosis.
A negative test by HIV RNA PCR almost rules out HIV.
However a positive test by HIV RNA PCR is not confirmatory that a person is HIV positive as almost 5% of the patients may have a false positive test result with this test because of contamination.
In view of that a final screening test for HIV antibodies at 3 months is routine and is considered to be conclusive.

To summarize: You may take HIV PCR RNA at 2 weeks rather than at 9 days. This test if negative almost rules out HIV. However, a positive test is not confirmatory of HIV.
The recommendation is to take up a screening test at 3 months for HIV antibodies and that would be conclusive.

I have repeated the combo today. today is15 days and 8 hrs from exposure . does this tell me any thing . the result was negative

Hi.

Negative PCR RNA test is almost a surety that you are HIV negative.
It would be better to be double sure but with a separate test i.e an antibody based test (ELISA or Westren Blot). That would be conclusive.
You can take up this test at 12 weeks or 3 months of exposure to cover for the window period of detection by this test as in some individuals antibodies to HIV may take up to 3 months to appear, though in most they would appear by 1 month.

Ok. I though you were mentioning about RNA PCR negativity.
The Combo should be ideally done at 4 weeks, therefore you need to repeat at 4 weeks. It is 99.8% sensitive at 4 weeks and 99.9% sensitive at 12 weeks.
At this stage PCR RNA result is more reliable because it gas a short incubation period. Anyway you will get the PCR RNA result after 5 days and if it comes out to be negative then it certainly would rule it oout
Combo is more reliable at 4 weeks.

I made XXXXXXX hiv Pcr on 15 which is after 15+ days also it came to be negative . so what is the status as per the current situation and the other tests made earlier do I have to repeat any thing or shall I do any other tests in futue or is it now conclusive since a pcr was done on day 8+ and a combo also .then on day 11 combo then pcr and combo on 15+ days in addition to the stds they were repeated on both the 8+ and 15+ . all tests were negative

Hi.

Negative PCR RNA at 2 weeks almost certainly rules out HIV. PCR RNA test is however not routinely used for screening of HIV infection. It is expensive and that is one reason, This test is mainly to monitor viral load in those who are HIV positive.
As I told you that HIV Combo should ideally be done at 4 weeks for it to be taken as reliable. You may take it at 4 weeks and if negative (most likely would be!)that should be the end of it in your case because 2 negative tests (PCR RNA and Combo test) at appropriate time intervals after sexual contact are a certain evidence that your HIV status is negative.
Only in those who are initially screened with an antibody based test (Rapid Antibody based ELISA test), a final screening test for HIV antibodies at 3 months is routine and is considered to be conclusive.
