Question: Sir i went to prostitute 2 weeks back. She done oral sex to me and i kissed her lips.other than this nothing happened.after weekdays i got severe fever and my doctor conducted most of the blood tests and nothing was found.I got a doubt n gone for vdrl test which came as positive 1:8 ration.but i dont have any symptoms of syphilils . Now i am worried about HIV.....as i effected with VDRL is there chances of effecting with HIV.I had only oral sex.Will HIV spread thorugh ORAL sex.Pl clarify..what are all the tests i should undergo to find any other STD's in my body.Pl clarify

I can understand your concern for the possibility of various STD s after unprotected oral sex. I can understand your level of stress after the incidence.

First and foremost to reassure you that HIV is not generally spreaded by oral sex. Though other of the STD s can get contacted by oral sex but not HIV. So first and foremost try and relax for HIV.

In your kind of cases I generally go for STD profile at 4 weeks of period which tells most of the STD s like gonorrhoea, chlamydia, trichomonia, herpes and syphilis.

For HIV there are two types of tests the one with antibodies which is done at 6 weeks and then is repeated at 12 weeks. The other which detects the viruses itself and can be done any time after 2 weeks. This test is known as RNA PCR and is done at all the reputed labs.
In my view your HIV test should come negative as oral sex is the least common known mode of infection.

I have studied some where in the net that 3rd stage of syphilis symptoms are fever,neck imbalance,joint pains..which all i am suffering now..and last year exactly 16 to 17 months ago i had sex with prostitute and later i gone for HIV tests 3,4 time which all shoes negative.But after oral sex that time i have got small red bumps on penis and some white fluid discharge happened.Later..every things was fine with out any treatment. Now again after intercourse 2 weeks back i got mouth ulcers and fever,headache,jointpains...Pl suggest what kind of treatment i should undergo and even my vdrl test is positive with 1:8 ratio.

I can understand your all the concerns. As I can get form your query you might be getting the symptoms of syphilis but not of 3rd stage but of 2nd stage.

I suggest you to meet a dermatologist near you and tell him/her all your symptoms. If I were your treating dermatologist then would have prescribed you Injection Penicillin as soon as possible. It is the drug of choice for your kind of situation. Syphilis is very well treatable condition and should not be worried about the same.

On humanly ground I suggest you guide your escort (the lady with whom you have had sex with) to take a prophylactic dose of Penicillin. It is she who is on greater risk for syphilis rather than you. I beg your pardon for the same but many a times we have to see the human ground that the other partner must be treated.

Simultaneously I suggest you to go for RNA PCR for HIV. Rest for HIV you should rest assured I do not think you wold be positive. Keep your moral high, avoid sex and get well soon.

dear Sir
I consulted doctor and he said he will give pencillin injection tomorrow..
i am using medicines from week days for my feverish symptoms but still they are not in control.
after taking pencillin will all my sysptoms go down and can i be normal person like earlier.i am very much worried about my health.Kindly suggest.

Today i have taken pencillin injection but while taking that the lab person over there said now a days vdrl and syphilis are very rare and said directly leading to HIV..after listening to this i was very much tensed and want to get some clarity from you.
as said i had sex with escort 1st time 1.6 yrs back with condom but she done oral sex to me and after some days i got symptoms like red bumps and rashes and after some days they turned off and never i got again.
and recently again i had oral with other lady on aug 1st.after that on aug 10th i got fever symptoms.and as said earlier i got vdrl positive with 1:8 ratio.
Now my confusion is because of which intercourse i got vdrl is it for last yr or recent one.
Along with vdrl i tested for hiv which shown negative.But on aug 1st i only kissed her lips and she done oral sex to me.
Pl clarify whether is there any chance of HIV as that lad technician said.Pl clear my worries

For your further queries-

As per your history the cause of positive VDRL can be due to the oral sex 2 years back.

VDRL can not come positive just after 2 to 3 weeks of incidence. It takes multiple of weeks to months to come positive. As you have given a history of some lesion with discharge after the intercourse 2 years back, so adds support to the same.

I presumptively have given you suggestion to go for penicillin as you are out of XXXXXXX I would like to confirm VDRL further by a speculated test called as TPHA or treponema pallid um hemagglutination assay. It is the most confirmatory test and done at all reputed labs of XXXXXXX

You just check at your place whether this test is available or not.

For the possibility of HIV- just relax and do not pay any attention on vague talks anyone. Both HIV and syphilis are totally different conditions. I have suggested you the treatment presumptively so that if there are any chances of syphilis then can be controlled at the earliest. A further TPHA can be done to confirm the exact situation. So do not worry and try to relax first.

If at all you are worried about HIV then can go for RNA PCR.

Thanks for your valuable answer and support.
As you said vdrl cannot come positive with in weekdays of time and its because of my 1.6 yrs back sexual intercourse then after than i have gone through hiv test 3 times and recent one is 5 days back which all report says its negative and i have even donated blood 2 times in last 6 months.hence i can be happy that i dont have hiv.
But my fear is as i am vdrl positive i feel i am having chances of effecting HIV.as you said both syphilis and hiv are different cases hope i dont have any chances of effecting with HIV.
But my 1st symptoms like red bumps on penis, and white discharge around penis were noticed only after weekdays of sex 1.6 yrs back.
And i would like to know how syphilis is spread at the time of sex and why cant HIV spread.
Pl clear my worries and help me in building my confidence back.

Syphilis can be spread by genital or oral sex. If the body fluid of any infected person comes in contact with mildly abraded surface of non-infected person then the organisms of syphilis gets an entry into the body. These organism then spread all over the body and body mounts an immune response against it.

If it is at the genital contact of non-infected person then there is formation of sore which is known as primary chancre or sore.

In such cases we do a VDRL which if comes positive then do TPHA as described before. if the facility of TPHA is not easily available then we give start the course of penicillin till we see the result of TPHA.

As syphilis is totally different form HIV so it is not possible that if a person is infected with syphilis will get HIV or vice versa. Though it is with all the STDs that these can increase the chances of one another but it is not possible that if a person is infected with syphilis then will must get HIV too.

So just relax about anything serious. Get your RNA PCR and TPHA done at earliest possible to relive all your tensions.

Dear sir
There are three diff types of RNA PCR in labs..Counts and all.
Suggest which is the correct test.
As you said syphilis is transmitted through body fluids if the person have HIV..then is there chance of transmitting HIV through the same body fluids.
What are the other tests i should go for confirming other STDs.
I have already taken a dose of pencilline and doctor suggested for other dose after 2 weeks.
As i tested only 5 days back for hiv along with vdrl in which vdrl is positive and hiv is negative.is it required to go for RNA PCR again.
Kindly suggest

For your further queries-

You can ask the lab people to go for most advanced test available in the lab. All the tests for RNA PCR are correct and it varies in only either company or its resources. Even if there is confusion then can take the name of all three and send it to me so that I can guide you.

As I said both syphilis and HIV are different entities so it is not necessary that if a person had syphilis then HIV is must to be transmitted. it depends on the HIV status of a person to spread HIV or not. Syphilis is more easily transmitted than HIV. So do not just intermix each other and do not worry about the same.

Though thre are so many tests which can be done but to limit to lesser number the other of the tests which can be done are-

Routine blood counts with IgM and Igg for Herpes simplex type 1 &2, Hepatitis B, Urine examination complete for routine ad microscopy.

For the test of HIV and VDRL- if you have gone for RNA PCR 5 days back then there si no need to repeat it. But is it was not RNA PCR then to know the best status it is suggested to go for RNA PCR or have to wait till 12 weeks in case of antibody tests for HIV.
