What Are The Chances Of TB Infection After Renal Transplant Operation?

Question: Respected Sir,
I had renal transplant on 27 Feb 2015.Everything is going well.Yesterday i came in contact with a person who is sufering from Mediastinal lymph node TB.Unfortunately i was not wearing the mask at that time .I am really afraid ,will i also get TB.What needs to be done now?I also had TB in 2009.Please suggest something sir.Will i loose my graft due to this?

No, You will not lose your graft because of this.

Detailed Answer:
Hi XXXX,

Mediastinal lymphadenopathy is generally not a transmissible disease until-unless the patient also has cough and sputum.

If the patient is on treatment with anti-tubercular therapy then he becomes non infective ( that is- he cannot transmit the infection by aerosol) in 15 to 20 days.

Also important is to note how close is the contact. suppose he/she was with you for 5 minutes talking to you sitting at a distance of few feet then chances are less of transmission. but if the person was talking to you from less than a feet distance and for a long time then chances of transmission are more.

So, after understanding all these facts you can judge yourself that what could be chances of transmission, which i feel would be rare.

But then also, even if you are exposed to TB bacteria, all exposure do not lead to TB disease, and there is no way to predict who will develop disease. i would not recommend prophylaxis.

So in my view you should rest assured and forget about the incident. Meanwhile be vigilant for any fever.

And You will not loose your graft because of this
