Question: I had ovarian drilling done on 01.09.2014. I have conceived post surgery. My LMP was 01.09.2014. On 21.10.2014, there was some spotting ranging from brown to red. As per the TVS Sono on 21.09.2014, my gestational sac was 5.8mm which corresponds to 4 - 5 weeks gestational age. But my HCG count on 22.09.2014 was 1110. So my gynaec suggested me to take hucog 5000 IU. Will I be able to keep the pregnancy? Is it a alarming situation?

Thanks for writing to us. Followings are my comments:

1) I have reviewed your attached report. Gestational age (ultrasound measured) DOESN'T correspond with last menstrual period date. It is more than 2 weeks lag - NOT normal.

2) At this stage (7 to 8 weeks), yolk sac, fetal pole and heart beat should be visible through Trans Vaginal Ultrasound scan. However, these are ABSENT.

3) Late conception is the ONLY possibility to be able to continue this pregnancy. You should repeat the scan to confirm the same. Otherwise, chance of healthy pregnancy is quite LOW in this case.

4) Possibility of MISCARRIAGE is quite HIGH with blighted ovum. Therefore, you will be prepared for the same. In the mean time, you should continue prescribed medication and Injection to get some result.

Thanks for your response.

1) I have calculated your gestational age on the basis of given LMP date in your query (1.09.2014). It is MORE than 7 weeks.

2) Trans Vaginal Ultrasound scan (TVS) DOESN'T correspond with your date. It reveals only gestational sac WITHOUT embryo and yolk sac, i.e. 4 weeks Gestational age.

3) I have mentioned earlier that you should repeat the scan to rule out LATE conception. Otherwise, chance of healthy pregnancy is quite LOW. Kindly, consult with your treating doctor regarding this.
