Thanks for writing to us.
Travelling with a 3-4 month old is challenging depending on the mode, duration and length of vacation.
I would advice you to

1. Keep first aid kit ready with main medicines like for fever, cold , vomiting. keep your previous prescriptions along with you.
2. Keep enough stock of formula,bottles if you are giving already , diapers, cloth, separate diaper bag.
3. Air travel needs sucking at take off and landing to avoid ear pain.
4. Car travel needs infant car seat in the rear.
5. As far as possible avoid crowded places and stick to less crowded spots.
6. Ensure enough clothes if travelling in this weather esp cap, gloves, sweaters, etc. Need two layers for outside cold weather.
7. Keep enough toys for entertainment
8. Enquire about pediatricians in the visiting place and their timings

Keep trip short and return safely
