Question: My husband who is 73 yrs old has been on ACTONEL for a number of years and has had no side effects. However, a recent visit to the dentist reveals that he has an infection in two of his remaining teeth (he wears a denture and it is attached to these teeth) and the dentist has ordered a blood test as he has been told it can cause complications in the jaw bones if extracted - he may need to visit an oral surgeon. How long does this drug stay in the system? Dr. has told him to cease taking it and to wait at least 2 months before any work is done? The dentist believes the drug can stay in the system for up to 10 years. Could you please give us some advice.

Thanks for your query, I have gone through your query.
If you are getting the teeth removed, you have chances of bone getting necrosed since you are taking bisphosponate (actonel). So you nee to get the CTX test done before extraction.
Elevated serum concentrations of isomerized C-terminal telopeptides of type I collagen have been reported for patients with increased bone resorption.
A stratification of relative risk is seen as CTX values less than 100 pg/mL representing high risk, CTX values between 100 pg/mL and 150 pg/mL representing moderate risk, and CTX values above 150 pg/mL representing minimal risk. The CTX values are noted to increase between 25.9 pg/mL to 26.4 pg/mL for each month of a drug holiday indicating a recovery of bone remodeling and a guideline as to when oral surgical procedures can be accomplished with the least risk. so consult your general physician who has prescribed actonel and oral maxillofacial surgeon and take opinion and proceed with extraction. If you are under going extraction, you need to get hyperbaric oxygen therapy done during extraction.
