What Are The Side Effects Of Giving Combiflam Suspension To A 32 Month Old?
Question: My son 32 months old took 50ml of combiflam suspension which contains 100mg of ibubrofen & 162.5 mg of paracetamol per 5ml. Yesterday at 5pm. Now its 10 am nextday. He is fine and active. We have given samples for LFT, CBP, CUE, Electrolytes testing. When should I know he is safe of side effects. How many days the effect of medicine is gone?

Hi...it will take 3-7 days for him to completely come out of the effect.
The total dose he has taken is 1620mg of paracetamol. This is very high for his age.
Usually paracetamol affects the liver and the immediate side effects will be jaundice and elevated liver enzymes and altered sensorium.
There are 2 different types of paracetamol metabolism in the body based on the genetic make up - it is -
1. Slow acetylators - complications are more likely
2. Fast acetylators - complications are less likely
As - even with such a high dose nothing has happened till now, he should be in the fast acetylators groups. That means - he will be metabolizing the drugs very fast and putting them out of the system fast,. So the complications will be less.
It would be useful for me to guide you if you can get back to me with the follwing test reports -
1. Liver function tests.
2. Prothrombin time.
3. Serum paracetamol levels

Hi...you need not worry. The total dose of paracetamol is coming to 70mg/kg/day. But the actual prescribed dose per day is 90mg/kg/day. So the cumulative dose is less than per day's dose (I mean the ceiling dose). So he will be normal. Do not worry.
The 6 images you have uploaded are the same. It is urine routine. There is some increased pus cells, but it is normal in viral illness. So do not worry.
I suggest you get the following investigations done -
1. Liver function tests.
2. Prothrombin time.
3. Serum paracetamol levels
4. Urine culture.
5. Blood culture.
Kindly get back to me with the reports.
