What Are The Signs And Symptoms Of Advanced Glaucoma?

Question: I went to the Ophthalmologist and they mentioned that I had cupping of the optic nerve but no glaucoma after studying images 2 years apart. In addition, my IOP was 7MM. Should I worry? Also how quickly does glaucoma progress?

I have gone through your query with diligence and would like you to know that I am here to help. There are 3 components that make up glaucoma, they are:

1. Increase in intra-ocular pressure
2. Visual field changes
3. Changes to the optic nerve

When the IOP is normal, but the other 2 components are present, a diagnosis of normal pressure glaucoma is made. You mentioned cupping of the optic nerve (number 3 on my list), please check with your doctor for any vision field changes (e.g blind spots in your vision).

If blind spots are present, then your diagnosis is 'normal pressure glaucoma'. So please confirm or rule this out first. For the time being, there should be nothing to worry about, speak to your doctor about the initiation of medications to help improve the health of your vessels (which will contribute towards preventing progression of the cupping).

Glaucoma has different types, many different types. Seeing that you are a your individual, the types of glaucomas that can occur at your age can take a few years to develop, but I cannot state exactly how much as it again depends on the type.

Thank you doctor, I also found out that my father and paternal grandmother have cupping with CD ratio of .8 in the right eye which is the eye i have cupping in. Could this be hereditary physiological optic nerve cupping?

Yes of course; hereditary optic nerve associated changes are seen quite often, and unless they present with symptoms causing discomfort, they are really nothing too serious to worry about.
