Question: What are likely causes for normal RBC, low hemoglobin and low hematocrit. RBC=4.7 Hemoglobin=9.7 and hematocrit=31.6. Will be getting more bloodwork done next week: Fe, TIBC and Ferritin level.

Low hemoglobin with normal RBC is called Normocytic normochromic Anemia. There are certain conditions which can cause this kind of anemia.

Common among them are:

1. Blood loss : This can happen due to blood loss in motions , urine or excess menstrual flow.

Hence I recommend tests to detect blood loss . These tests are : Urine for RBC, Stool for occult blood.

2. Blood destruction: Lysis or destruction of RBC can occur inside the body due to certain conditions like Hemolytic anemia.

For the evaluation of hemolytic anemia I recommend
Coombs tests, serum LDH, Retic count, Serum Vitamin B12 levels.

3. Thyroid problems: Less production of Thyroid hormones can cause this kind of anemia.

Hence kind get Serum TSH test done.

4. Kidney problems: Kidneys produce hormone called Erythropoitin . This hormone is necessary to stimulate the body to produce RBC. Certain kidney problems will cause less production of Erythropoitin hormone.

Hence I recommend serum creatinine and serum Erythropoitin levels.

Kindly get the recommended tests and revert back with reports.
