What Cause Difficulty In Getting Orgasms?

Question: My partner is a 46 year old male. We have been together for 2 years. We have both enjoyed our sexual relationship. He has no problems getting or maintaining an erection or ejaculating. We have sex on average 2 times a week. In the last 3 months however he is ejaculating but getting absolutely no feeling whatsoever. He enjoys the lead up to sex, can feel himself about to ejaculate but then no sexual feeling or release. He is getting distressed and thinks he is letting me down. He is getting to the stage where he will please me but is quite happy not to go further as he gets no satisfaction. Can you please point us in the right direction ?

From the description of your husband's sexual problem, it appears that he is suffering from a Pleasure Dissociative Orgasmic Disorder (PDOD). In this disorder, the person is able to experience the physical sensation of reaching the point of ejaculation, but unable to experience the psychological feelings of pleasure of the orgasm.

There can be several causes for this disorder - both medical causes and psychological causes. In your partner's case, since this has been of a relatively sudden and recent onset, medical causes should be first ruled out. Hormonal imbalances such as low Testosterone or high Prolactin, neuropathies, vitamin deficiences or excesses, etc. are some of the common causes.

So, my advise is that he needs to have a detailed medical evaluation to find out the underlying cause of his sexual problem. This will include a physical examination (including a neurological examination) and certain blood tests like hormone levels, etc. Further treatment will depend on the cause identified.
