Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.
Since he is having severe pain and persistent vomiting ( with the risk of getting dehydrated), take him to the nearby hospital to relieve the symptoms first.
Since this has been recurrent I need some more information before I can assist you.
1. When did all these start?
2. Did it start after the appendectomy surgery?
3. Can you describe the pain - is it uniform throughout or keeps waxing and waning?
4. Which part of the abdomen does he feel the pain?
5.Any abdominal distension?
6. Does he feel better after vomiting?
7. Any weight loss?
8. Any other medical illness?
9. What tests done so far?
10. What medications is he taking?
11. Is he getting all these symptoms even after excluding dairy products?
Get back to me with these information and I would give my opinion.
Regards,
Dr.R.K.
Appreciate your investigation on this, please find below the response to the questions.
1. When did all these start?
(Response)– off long the stomach gets upset. If I recall to the best of my knowledge, since I moved out of my house (close to 10 years) the stomach issue seems to be prevalent.
2. Did it start after the appendectomy surgery?
– I guess, the situation has aggravated since my appendix removal
3. Can you describe the pain is it uniform throughout or keeps waxing and waning? – this time it was concentrated in just one area, upper side of the stomach (left side), otherwise not specific to one area
4. Which part of the abdomen does he feel the pain?
- upper side of the stomach (left side),
5.Any abdominal distension?
– nothing seems to be visible
6. Does he feel better after vomiting?
– yes, a lot of relief

Endoscopy advise
Detailed Answer:
Hi,
Thanks for providing the additional information.
Nice to hear that he is responding to the treatment.
I presume the 13 kg weight loss is intentional.
He is having signs of dyspepsia and irritable bowel syndrome. The gastroenterologist has prescribed medications for these two issues and he is responding well to the treatment.
But since he is having recurrent vomiting and that he feels better after vomiting, obstruction should ruled out.
I would advise an upper GI endoscopy first to look for lesions in the stomach.
Discuss with his gastroenterologist about getting this test done.

Dr.R.K.
