Hello,
I have gone through your question and image.
The prominent vessel visible is superficial vein rather than carotid.
The swelling may be inside and pushing carotids if it's deep. Ultrasound will clearly show the tumor. Mri is another investigation which clearly delineate the extent and nature of tumor.
The surgery in such cases are done by general surgeon, onco surgeon or ENT surgeon depending upon nature and extent of involvement.
Headache may or may not be related to this problem.
Is there any sleep disturbances, vomiting, sensitivity to light or sound?
Duration and severity of headache?
Any aggravating or relieving factors?
Any diplopia, visual abnormalities?
