Thanks for asking on HelathcareMagic.
Our nerve supply is such that branches of the same nerve supply portions of different organs, both internal as well as external. So, when one branch is stimulated, the body cannot often cannot distinguish which branch the signal is coming from, and the sensation is generalized to the entire area supplied by the nerve. This is the reason that when the blood supply to the heart falls (it happens during heart attack), the pain is felt in the left shoulder and arm. In your case, a similar thing is probably happening. This is known as referred pain. This does not mean that the shunt needs to be removed. What you are feeling is the referred sensation from the shunt.
Hope that I have been able to clarify your doubts. Feel free to write back in case of further queries.

Referred pain seems the explanation that fits
Detailed Answer:
Yes, I think that the sensation is a referred sensation. There can be a component of sympathetic overactivity as well. There is a segment of our autonomic nervous system (which deals with internal functions without us even realizing) called sympathetic system which runs along the blood vessels and its overactivity could lead to changes in blood pressure and heart rate. The decrease in blood volume could also precipitate such a blackout, passing out or even convulsion.
