Miconazole would help a vaginal yeast infection if present.
It would not help other types of vaginal infections or infections of the urinary tract (bladder, ureter, urethra or kidneys).
Bacterial vaginosis can be associated with a fishy odour. It is possible that this may be the cause of your symptoms.
This is treated with topical creams such as metrogel or oral tablets such as metronidazole, cleocin, tinidazole.
This is not considered a sexually transmitted infection.
You can consider a vaginal swab to to determine if this or any other bacteria is present.
Urinary tract infections would normally present with: frequent urination, discomfort on urination, change in the odour or colour of urine, blood in the urine.
Normally the discomfort on urination and frequency are present at least.
The odour of urine can be influenced by foods or drinks used or medications.
You may want to go over anything that you may using that may be contributing to the problem.

Cymbalta can cause an abnormal urine odor but this is a very rare side effect that is only seen in 1 to 0.1 % of persons so it may or may be involved since the chance of it occurring is low.
Metformin, omeprazole, singulair, losartan, simvastatin and meloxicam are not associated with a change in odor though meloxicam, simvastatin and losartan can sometimes cause a change in appearance of the urine.
The B vitamins can also be associated with an urinary odor in some persons so they could also be contributing to the problem
