What Causes A Painful Lump Between The Buttocks?

Question: Yes. My boyfriend was released Wednesday from the hospital after having multiple, bilateral PE's. Before he was released, he had a BM, and he strained to do so. He immediately experienced soreness and now has a painful lump between his buttocks. He says it is very painful and hurts more when he has a BM. It is just a lump, not red, no pimple-like head, and it is probably quarter size. Do you have any suggestion as to what it may be?

Firstly, please let me know what medicines he is on. Due to the PE the Doctors would have put him on some blood thinners or Anticoagulants like Actirom or Warfarin.
Does he have constipation or hard stools?
Does he bleed during or after passing stools?
The lump is probably a blood collection around the anal area or a hematoma caused due to having Hemorrhoids from before.
Please apply warm compresses over the area and start some stool softeners like Syrup Lactulose 20ml at night.
Do let me know the details so I can advice further.

Follow up: Dr. Rishi (33 minutes later)
He was on Heparin and Coumadin in the hospital. The past three days, he has been on Lovenox injections and Coumadin. His blood is now in the therapeutic range, so he will proceed with Coumadin only.

He has not had any previous problems with constipation or hemorrhoids. This lump is mid-way from the anus to the tailbone on one side only. It began to hurt and continued after the first BM he had in the hospital. There was nothing before, and there has been no blood. He was constipated in the hospital from all the pain killers he took.

It seems like a Hematoma from some bleeding during passing hard stools.
You should get him checked up by a Surgeon for this.
If it is a Hematoma it will get resolved in 2 to 3 weeks time.
Follow the treatment that I have advised and get him examined and he should be fine.
