What Causes A Painful Sore On The Nipple After Taking Azithromycin?

Question: I am female, age 50 & no longer get my cycles. Yesterday I woke up with a sore pain int my left nipple.
Today it is hard. My right one is normal. I started a Z-pack yesterday evening for a dental issue. What could it be?

I have no history of this issue. I had not taken the drug before this situation happened. I'm taking the drug now, I started it last night, for an ulcer on my tongue that was caused by a sharp edge on my tooth.There is no change in tempurature, no itching nor redness. It is hard on the somewhat underneath part though.
