Question: What does it mean if a pediatrician says a four year old girl who appears to have a dark blue/purple bruise on her rectum says he does not believe it is caused by Abuse/trauma but rather it is "blood pooling"?

Unless you are talking about Mongolian blue spots which are present since birth, bruises would have some underlying cause. It could be due to accidental or non-accidental injuries or could be due to coagulopathies. The cause needs to be elicited through careful history talking, background checks as well as investigations as deemed required. It is possible that your doctor has ruled out abuse (physical or sexual) based on his findings and correlations. Possibly he suspects some coagulopathy that could have led to pooling of blood in dependent areas.

You will need to discuss with him about his findings that make him think so.
