What Causes A Ring Like Rash On The Neck In A Child?

Question: Hi,
I found a small dark ring kind of thing around my 3 year old son's neck. Having read somewhere that it is a sign of diabetes, I am panicked. I have attached the pictures. Can you please check and let me know if it's pigmentation or is it something to be concerned of?

Hi
Thanks for being at healthcaremagic
I can't see any pictures. Kindly upload them again so that I can have a clearer picture of the condition and can guide you accordingly.
Moreover what is the duration of this ring?
Is it associated with any itching, burning or any other symptoms?
Is your child having any other ailments or taking any other medicines?

Hi,
Have attached the pictures. Kindly have a look.
I am not sure about the duration of the ring. I noticed before few weeks. It isnt itching. He has cold right now and an allergic reaction and has been on allegra for the past 8 days. But this ring has been there for a longer time.

Brief Answer:
secondary hyperpigmentation or atopic dermatitis or acanthosis nigricans
Detailed Answer:
I have reviewed the attached photographs and can appreciate some hyperpigmentation on the back of the neck.
There are few possibilities for its occurrence:
Either as a secondary hyperpigmentation to some eczematous or allergic reaction in the past ; or due to atopic dermatitis which is a kind of endogenous eczema associated with dry skin, itching and rashes on other parts of the body ; or acanthosis nigricans which is associated with hypothyroidism and insulin resistance in the body.
Is your child allergic or is he or anyone in the family having history of dry skin, allergic rhinitis or bronchial asthma?
Is their any family history of diabetes or hypothyroidism?
To rule out acanthosis you can get your child's fasting serum insulin and thyroid function tests.

Hi,
He has hyperpigmentations on his leg from a lot of mosquito bites. His skin is very dry.
Also, his dad has allergic rhitinis but he no longer suffers from them. Maternal and paternal grandmothers have hypothyroidism and diabetes though myself and his dad are not.

Brief Answer:
Hyperpigmentation can be related to eczema
Detailed Answer:
The hyperpigmentation can be related to mild eczema.
If I was his treating doctor I would recommend him to apply mild steroid like Clobetasone cream on the affected areas twice daily along with frequent and liberal application of a moisturising lotion on the entire body for 10-14 days.
Moreover just to rule out acanthosis you can get his thyroid function tests and serum insulin levels checked.
