Question: I have a small bump when the foreskin of the penis is pulled.what it could be?

I have gone through your query and picture. It looks like fordyce spot. Fordyce spots are modified sebaceous prominences and are harmless.
When you noticed this?
Did you had recent unprotected sex?

Fordyce spots can be more evident when there is more sweaty and greasy environment on genital area.
Using aloe vera gel for local application and taking garlic and anti oxidants will be helpful to get rid off this.
Your picture is not so clear and there is glare of light. If you provide another picture with more clarity it will be more helpful.

Hi Doctor,

I have updated a new clear pic.

I noticed this bump on Friday evening.

Yes I had unprotected sex on last Tuesday.This is the only history of sex in the last six months.
