Brief Answer:
It seems like a Boil or an Abscess.

Detailed Answer:
Hello,
Welcome to Health Care Magic and Thanks for your question.
Please do not worry.
What you describe sounds like a boil or an abscess.
It would be having a small pus collection inside which is leading to the pain. There would be redness in the lump area and the temperature of the lump would also be higher than the surrounding temperature.
This would confirm that it is a abscess.
It would need a course of antibiotics and also maybe a drainage of the abscess.
But you would need to get it examined before that can be decided. Please get it examined by a Doctor or a Surgeon for the same.
There is absolutely nothing to worry about.
It would be treated simply.
Take care and do let me know if there are any questions on this issue.

You can't see it. Its painful and under his skin above the collar bone. Are bad lumps painful? He won't let me feel it because he says it hurts.

You can see it under the skin too

Brief Answer:
Please get it examined.

Detailed Answer:
Hi,
Thanks for the info.
You said you cant see it. In the last line you said you can see it under the skin.
Painful lumps are mostly infective. As I told you - Abscesses or boils.
Please do not worry and get it examined by a Doctor or a Surgeon.
Antibiotics will need to be started first. It would be fine soon.
But dont ignore it. You could start Tab Augmentin 625mg thrice a day and Tab Diclofenac twice a day all on full stomach. But it would be best to get it examined first.
Take care and do let me know if I can help you further.
