What Causes Abdominal Pain Followed By Fever And Hives On The Body?

Question: hi...

my 6 year old son started feeling poorly on wednesday...

Weds night: not feeling too well

Thursday: abdominal pain, upper leg pain, lying in a ball, lethargic, Fever
went to toilet, and abdominal pain went down.

Friday:
fever gone, lethargic, slept most of day, not eating hardly anything, vomited, vomiting water

went to doctor on friday morning
- did blood test, said score was 9, and no evidence of bacterial infection
- did appendicitis test, was fine
- no evidence of meningitis
deduced somach flu, and said would clear up without further treatment.

Sat:
morning: vomiting water, lethargic
been feeding him small amounts of fruit including strawberries
fed water via a spoon all day
went to toilet to pee, not too yellow, not pooed

afternoon:
hives on whole body
lethargic

Sunday morning-
lethargic
hives disappearing from body, reappearing on feet
drinking ok, feeling ok... up and about...
fed pasta + bread

Sunday Afternoon
hives returned on face - looks awful + some of body... was up and about for quite a while, then went to rest
Asleep now but saying mild abdominal pain

Thoughts ?

By what you quote I deduce that it is total 4 days of illness. Apart from fever the associated symptoms are stomach pain, occasional vomiting and rash now which is looking like skin allergy. He is active when there is no fever.

By the image I see that he has it on face, but you quote that he has it all over the body and appearing / disappearing.

Considering all the above findings, I suggest these possibilities -

1. Viral exanthematous fever - it is a viral illness which while subsiding will cause rash over the body. Usually it is called viral haemorrhagic fever and these groups of illness would usually complicate when fever is subsiding on 5th or 6th day.
2. Toxic urticaria or viral induced urticaria.
3. Streptococcal infection - I suggest you get a rapid test done.

A few more bits of info..
- we had a blood test on Friday, and there was no evidence of bacterial infection.. (Results were =9).. Bacterial infection would = 40

Should we do another bacterial test today ?

He just had a pee
- clear and looks normal
He just had a poo
- solid

He's asking for Cheese.. Should I give him cheese to eat ?

He has had a runny nose for months caused by hayfever.. He also has had some ezcema in the past, but its gone now.

Hi...Thanks for trust in me.

Fever of few days could as well a viral illness.

Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all....that means it is not bacterial infection.

Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Paracetamol can be given in the dose of 15mg/kg/dose every 4-6th hourly that too only if fever is more than 100F.

Coming to your questions -
1. You need not go for another laboratory test.
2. You can give him anything he is wanting to eat. Usually kids will eat less, when they are ill, so I suggest you give him anything which he asks for (including cheese) and is palatable for him. Only thing to avoid will be spicy food as it might cause gastritis.

XXXXXXX - we've been in the UK and Sweden, but it can't be the the scandinavian version as he was only in Sweden for a few days before getting ill.
XXXXXXX - 2 other members of the party felt unwell. My 2 year old had diarrhea for 2 days, and was a bit lethargic, but recovered quickly... The other member had a bad stomach for 5 days, and ate only porridge. Recovered now.


2. Toxic urticaria or viral induced urticaria.

XXXXXXX - my guess is that viral induced urticaria is the most likely.. are there any foods I should avoid ?... Every time I give him food, he seems to get hives.

Thank you for the information...The best clue among all of them is that- there were other family members who were unwell and are now recovering. It is definitely viral. This history is classical in viral illness affecting many family members.

Toxic urticaria is nothing but hives or some sort of rash which is induced by the invading virus. It is body's reaction to the virus. It will usually settle and is self limiting. You need not avoid any food specifically. But if as a parent if you are vey sure that only specific foods are aggravating his rash....better avoid them.
