Brief Answer:
Heartburn associated with GERD.

Detailed Answer:
Hello Dear,
Thanks for your query.
Have gone through your details and i appreciate your concerns.

As per your query 40 mg Nexium per day is just a initial dose and if you are not responding to it a high dose like 40 mg twice daily should be taken. Nexium has been tried at doses as high as 240 mg/day safely in patients.
So if your body is responding good to 40 mg dose you can continue taking it in higher doses.

You are suffering from reflux disease which consist of two phases-

first part is overproduction of acid in stomach(Nexium and other PPI's inhibits the secretion of acid from parietal cells of stomach) while second phase consist of reflux of acid back into esophagus from stomach(Nexium has no role is slowing down acid reflux hence ask your primary to combine domperidon (prokinetic drug) 30mg or Levosulpiride once daily with 40 mg Nexium. Results would be much better then 80 mg Nexium alone.

For maximum benefit Nexium should be taken empty stomach with just a sip of water and you should not eat anything for next 30 minutes).

For further relief antacid containing Oxethazaine +Aluminium hydroxide +Magnesium hydroxide should be taken 2 tsf twice a day. This would help neutralizing acid.
In case you have any other query feel free to ask.

Hope my answer helped you.
