What Causes Acute Abdominal Pain In Right Lower Abdomen With Fever?

Question: I have been having acute pain in my right lower abdomen with fever for about 30 hours. I have been slightly constipated but laxatives have relieved that symptom. It is most painful when going to/from standing and sitting. Very little pain when sitting. My appendices was removed when I was 21, I'm now 54. When I was 40 I had a hysterectomy but still have my ovaries. In 2010 I had an ascending aorta repair, this was due to a family history.

Thanks for writing to us with your health concern.
Have you had any unaccustomed exercise , unnatural sprain , sleeping or working in abnormal posture, bending etc ?
You could have pulled a muscle there.
Another possibility is a mild bowel infection ( gastroenteritis ) - usual symptoms include abnormal bowel patterns ( diarrhoea and / or constipation ), fever etc.
A stool exam would rule this out.
Also, an ovarian mass or twisting of the ovaries could cause this.
I would request you to have a formal gynaecological exam ( internal check up - per vaginum and per speculum ) and a pelvic transvaginal ultrasound scan .
At times, there might be adhesions / bands that form at the site of previous surgeries, and you have already mentioned two in the pelvis .
You could try taking OTC painkillers and a hot water bottle at that site, but please do have an evaluation, as the combination of pain with fever cannot be ignored.

The pain came on rather suddenly, I was fine until Thurs. evening and then it became more acute while I was sleeping. The pain has not become worse and today slightly better. It does feel like a pulled muscle but nothing out of the ordinary to cause it. When I press on the area, which is swollen, it feels like gas. I'm concerned that I will not be able to get an appointment in a timely manner. How long should I wait before going to the ER?

If you take a painkiller and then a hot water bottle, and are still not better in a few hours, please go to the ER.
Since it is better , albeit slightly, I think it is a pulled muscle or just gas .
Avoid eating spicy, greasy food and stick to broth / soup.
Take an antacid, even that might help.
