Question: My conspiracy-theorist schizophrenic 17 year old brother who we stopped from accessing the internet because of how it fueled his mind with negative thoughts asked me to download 'Hilary's America' for him to watch, should I allow him to watch such films? He is easily aggitated and obsessed with being right and always blames us for anything that goes wrong in his life like him dropping out of highschool even though we paid all his school teachers to come to the house and teach him in the evening, he absolutely refuses to sit with them. He only thinks of taking up hobbies like violin or mixed martial arts but would never leave the house to pursue them and only sits infront of the tv all days and bothers the maids. His IQ is higher than normal people. How do I behave around him? And what should we do as his family? He has been in and out of the hospital 3 times and is on

From the available description there is clearly one of the two things. One is that he is not taking medication properly. Secondly if he is taking medicine it is not working for him.

I mean to say there is need to take care of his medications as his current symptoms are indicative of active psychopathology.

Regarding movie yes you can allow him to do so as these thoughts are mostly generated from their own mind and rarely from the external environment.

I also request you to provide some of the responsibilitiesto the patient. This will assist him in some attenuation of symptoms.

What responsibilities do you recommend we give him without exerting too much pressure? He has repeat exams next month but he isnt willing to pick up a single book. Also regarding medication, he has been caught spitting them out when he thinks were not looking and is very defiant as he thinks it messes with his brain and his intellect, apparently he saw some videos and read research online that told him so.
Do you think we should admit him into a rehabilitation center, he has only been to hospitals where the most they would do was monitor his medication intake.
We live in Kuwait and theres not much resources on his illness and not many specialists so Im looking for rehab centers abroad in place like UK and XXXXXXX

Thanks for reverting back to me.

First I will start with treatment part. In my view he should be considered depot preparation of one or another antipsychotic. Currently there are many options for depot which includes Invega, Zyprexa, Risperdal, Haloperidol, Flupenthixol, Fluphenazine.

Considering his poor compliance I feel depot is better option for him as the drug remains in the system for a two to four weeks and this helps the patient to remain symptoms free.

Regarding admission rehabilitation center is better but it should be done only after management of his acute symptoms. So whatever you decide, decide once he is better so the rehabilitation process can be carried out.
