Regarding to the thyroid nodule i would say, there is no relation between it and high lymphocyte level or back pain.
The malignant characteristics of the thyroid nodules are: irregular borders, hypoechogenicity, microcalcifications, high vascularisation. You did not mention any of them, so the chances to be malignant are low. The presence of cystic components suggests for benign lesion.
The high lymphocyte level should be monitored, if they persist high after one week, further investigations should be done to exclude any chronic infection or other hematological diseases.
The fact that the back pain persists since 3 years, suggests for a chronic process such as: herniated disc. I would suggest an MRI of the vertebras where the pain is located.

Hello again,
The thyroid nodule is not caused by medication, usually there is a genetic predisposition.
The thyroid hormones would help to determine the thyroid function (rather the nodule is toxic or not) and the FNA will give of course information regarding the nodule nature.
The characteristics are not suggestive for malignancy but the presence of neck lymphnode (if it is not described as reactive in ultrasound) raises some doubts.
So, you should wait for the FNA answer.
