Question: Dear doctor i had a bike fall and while lifing it back...i had a bad back catch...mri shows L5 S1 disc extrusion compressing the thecal sac and the traversing nerve root...its 5 weeks now since the accident...
Symptoms;
intially it was only vack severe back spasms for a week but after that back problem completely vanished and but the pain started in my left calf musles when walking ...along with that a part of left feet towards last 2 fingers got partly numb(though 50% sensation is still there) few days i could walk without pain but few days severe calf pain...all the while i was taking muscle relaxants twice daily and NUGATRIP(amitryplene & pregabalin combo) once at night...now the issue is from yesterday the numbness has started in my left lower leg also upto knee on the back side of the calf....totally now i feel numbness in the left feet and calf both on lower side...i have underwent 10 sessions of traction and the radiation heat therapy on my back...can i get away without surgery given my condition by doing physio therapy with back exercises...and if surgery is necesseary which 1 is most appropriate...micro discectomy , open discetomy or laminactomy...my disc herniation is on only one side on tje left...
Second question is after surgery can i get rid of medicine nugatrip as i am tired of taking that medicine
Also what are mesures to be taken post operation..how soon can i get back to my normal work post operation

Thanks for posting your query.
The increase in numbness is suggestive of an increasing nerve compression. Surgery will help but it is not an emergency situation. You can try conservative treatment for 2-3 weeks. In addition to the medicines you are taking, lumbar traction, oral trypsin- chymotrypsin and oral steroid - Deflazacort can help in improving these nerve symptoms.
In case there is a gradual increase in numbness with decrease in muscle strength then surgery may be needed - Laminectomy is a better option in such a case.
In case surgery is done, you will need at least 6 weeks rest after the surgery. It will take around 6 weeks to resume the normal working.

as far as i have learnt..laminectomy is relatively a major surgery...cant i not do with micro discectomy or a minimally invasive discectomy or an endoscopic discectomy...

If the prolapsed disc is a minor one then only microdiscectomy will also be sufficient. It is the operating neurosurgeon who is the best person to decide regarding this according to the MRI scan and severity of symptoms.

thanks for the reply doctor...i also want to know about minimally invasive discectomy..(endoscopy or laser )which is suppoaed to be the most advanced techinque...is it succesfull...if so can i know any gud hospital in XXXXXXX where that procedure is done...kindly enlighten me

Laser discectomy is more advanced and the recovery will take lesser time after laser discectomy. The technique is successful but even after the procedure one has to regularly do back strengthening exercises to retard the degenerative changes in the spine.
