Brief Answer:
Fixed drug reaction is a possibility.
Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
Black patch underneath of shaft of penis extended up to the head of penis could possibly because of a drug eruption if you had any history of drug intake. This could be settled once you stop the drug, the discoloration may remain for months.
Sometimes discoloration is possible in normal genital skin which might not had been noticed earlier.
If you feel that your problem would fit with this things, then you can upload the picture (with good clarity) of the lesion, then we may able to discuss further.
It would not be anything serious.

Brief Answer:
Not seems to be a pathological lesion.
Detailed Answer:
Hi,
Welcome back.
I saw the picture sent by you.
It does not fit with Fixed drug eruption. I am able to see few brownish (Not exactly blackish) discoloration of the inner aspect of foreskin. It is neither due to any STD or HIV. It is almost a normal discoloration which you can ignore. If you have any discomfort with this discoloration, consult a Dermato-Venereologist for further help. Direct inspection of the lesion may throw more lights on the same.
