Hello,

painful hematuria is usually due to renal stones. The pain starts from the costovertebral angle (flank pain) but may move to the front and radiate to the genitals. Nausea and vomiting may appear. You shouldn't have fever!

Another possible explanation is a urinary tract infection (UTI). A lower UTI may present with frequency, burning or other kind of pain when urinating. Severe bleeding with clots is not common but it can't be excluded.
An upper UTI presents with flank pain and fever. Hematuria is also possible.

Finally pain and hematuria may be present in kidney or bladder tumors.

Careful clinical examination and appropriate laboratory investigation including a urinalysis, ultrasound scan of the urinary tract and other tests (depending on the clinical picture and urinalysis/ultrasound findings).
