What Causes Blood Discharge From The Penis?

Question: My husband, 62 years old and in apparently good health, awoke this am with blood on his underwear, apparently due to bleeding from the penis overnight. There were no external areas that appeared to be bleeding. He then urinated normally, with no apparent blood in his urine. He has had no symptoms related to pain, urine urgency or and other genitourinary problems. The only other pertinent history is that he had an ultrasound diagnosed scrotal hydrocele a few years ago. He had a routine physical exam with a normal rectal prostate exam last month. How concerned should we be? Thank you.

Thanks for your query and an elucidate history about your husband.
To recapitulate: Male/62 - apparently good health, woke up with blood on underwear - ? bleeding from penis overnight - no external area that can cause staining - then urinated normally - no blood in urine - no symptoms - hydrocele since few years - normal prostate exam last month - wants to know how concerned should you be ...
Well, as per the history you have provided this may be due to bleeding from the prostatic congestion or so. There is no history related to genitourinary system and the rectal prostate exam was normal last month.
Hydrocele is in no way related to the present problem.

I would advise him the following:
Send the urine sample for lab to see if there is microscopic hematuria or infection.
Blood - CBC, Bleeding and clotting time, PSA is prostate specific antigen
Ultrasound of abdomen, particularly for genitourinary system - prostate.
Clinical evaluation, examination of urogenital system, scrotum, prostate preferably by an Urologist so that further evaluation be done and cysto-urethroscopy can help to get a more accurate picture either to prove or to rule out any serious problem.
