Question: My aunty has lupus for 4 years and now she has bile duct cancer from the test its say she has colangio carsinoma,mestatic process,we live in indonesia the healthcarr is very poor, now shes jaundice, cant eat anymore only liquid food also pain in the back of abdomen.also just now she has blood a lot coming from her anus its like chicken liver, she tought she need to go to the toilet and after she almost fainted, i need answer what cause this?is jt bad stage already

As you say its cholangio carcinoma and is metastatic then its pretty advanced stage...even in the most medically advanced places, the prognosis is very poor.
Look, as the cancer grows, it spreads to nearby organs, and the bowel lies just next to it...it grows into that and also spreads to the adjoining lymph nodes which in turn compress the bowel especially the esophagus...so she is not able to eat solid food...also as it grows into the bowel, it also grows through the blood vessels as it needs nutrition to grow...in this process many vessels get injured and bleed, both inside and outside the bowel...the blood collected inside moves out of the anus either as XXXXXXX blood or clotted blood depending upon the rate of blood loss....also sometimes a piece of that growth can get detached and come out from anus as a piece of flesh...this ongoing blood loss makes the patient weak and anemic and they may even faint...which was the case with your aunt...
No one can do anything much at this stage...all that can be done now is to take care of her as much as you can in her last days...

Pain will be there as the tumour grows into other organs leading to distension of capsule of those organs...that capsule is pain sensitive...
I would give her morphine for pain. Alternatively you may try tramadol. But these are strictly prescription drugs...so see the local doctor for that...

It depends upon the blood loss and her hemoglobin level.
But as you had said that she is bleeding and has even fainted...so it implies that she is very much anaemic...that why they must be transfusing her...as you need to replace the lost blood else she may collapse...
