What Causes Blood In Sputum And Recurring UTIs?

Question: Dear Doc,
I have the following problems since last 1.5 Yr. Initially when i used to brush my teeth in morning, small amount of blood used to come, when i took the sputum out. it used to be once or two times of sputum throw. However the blood in sputum used to come at irregular intervals and that too in morning time only. I thought it might be coming from the gums / teeth and ignored.

After a few months, during brushing of teeth I felt like clearing my throat of sputum for about 10-20 minutes. afterwards this feeling used to subside. However whenever the toilet used to be cleaned with acid, the smell used to cause me more feeling of clearing my throat.

I visited my Doc almost after 8 months, The tests like X-ray, CBP, CUE etc were normal and he told me that I have Allergic Rhinitis, for which He started a Medicine Romilast-L 10mg, once in night time.
I felt some relief in the throat irritation and the blood in the morning sputum almost stopped.
However sometimes some dried blood used to be present in the nostril mucus, on some occassions.

I visited the ENT Doc, also, who suggested throat swabs and checked through Laringoscope. She said some swelling and redness is there in the throat area. I took a course of antibiotic also, durin which the throat irritation almost subsided but cam back after the medicine was over.

In between I felt some discomfort while passing the urine. Every time the urine is pale yellow and some burning sensation in the penis after passing of urine. I got Urine C/s done in which Escholi was detected, for which I took some tablets for 14 days and the infection was gone. However the discomfort and yellow urine continues. Again I got CUE done and recently Calcium oxalates are found, for which my doc suggested to take Noculi Syrup. I have just started using it and have to wait for 15 days to see what happens.

Dear Doc,
Please tell me if the above diagnosis is ok or it is suggestive of some other disease. I am very much concerned about my health. BTW I got Endoscopy, Barrium X-Ray Test, Sputum cytology tests etc done which were normal. I am Male, 40 Yrs age. I shall be very much grateful for your reply.

Hi, thanks for your query.
Sir you have 2 problem , one is blood in sputum, second is repeated urinary tract infections.
As a ent surgeon I would want to comment only on blood in sputum.
Analysing your symptoms , it feels probably you have allergic rhino pharyngitis.
Allergic reaction will cause mucosal congestion in nose and throat ,which leads to fragile capillaries in that area.
This may cause bleeding while sneezing/ coughing or blood in sputum,even while clearing the throat. Once the allergy is treated, congestion reduces and blood in sputum or in nose will stop.
Apart from this, chronic infectious disease / tuberculosis disease also can cause blood in sputum. Your blood work, chest x ray, sputum analysis , all are normal,
So chances of the disease are less.
You have completed a course of antibiotics also. But the throat irritation still persists.
If I were your treating doctor , I would have advised for allergic skin prick test todetect the allergen, and would have advised for immunotherapy shots for permanent cure of allergy.
Empirically I also would have prescribed romilast-L for a period of 3 months.
Avoid eating cold items and oily foods, curds, refrigerate d itemx, citrus fruits as it can trigger allergy. Your treatment and diagnosis is correct.
You dont have any other disease as far as ent is concerned.

Dear Doctor,
Thanks for your quick reply. As you have replied is it the first symptom of haemetemesis ? which means coming of blood from upper gastrointestinal tract. For this I got a Endoscopy / Barrium swallow tests done & the doc said that the gastrointestinal tract / esophagus are ok and blood may not be coming from there.
similarly as the sputum cytology and chest x-ray is normal, which shows the respiratory tract is also ok. Is it then the blood might be coming some where from nasal lines ?
I do not have any chronic disease / TB / Thyphoid etc. I had malaria sometime around 10 yrs back. I had undergone surgery for appendicitis around 20 yrs back and surgery for piles around 10 yrs back.

I do have allergy with curd & banana since i was 15 yrs old. Now also I never take these two items, as it boosts the esonophils in the blood and sometimes i have to take Banocide forte for this.

If I have allergic rhino pharyngitis, then what are the further tests which can be done ? what is the further course of treatment and what are the foods which has to be avoided ? BTW I am taking Romilast-L 10mg from almost 7-8 months, since I have started seeing my GP. He says I have allergic Rhinitis, which may not be curable. By hit and trial i have to find out myself which are the food or items which may increase my problem. He doesn't suggest any further tests. Also he says I may have to continue the medication till it subsides fully, may be life long also.

For the second urine related problem, I am trying to get an abdomen USG done tomorrow to check if I have got any stones. CBP / CUE is normal except Calcium Oxalates were found in the urine samples. From past one year I have pale yellow colour urine with some burning sensation / discomfort. I feel the urine output is less even i am trying to increase the intake of water. No proteins etc are found in the urine report. Please advice me on both the issues.

Hi, thanks for the follow up query.
Regarding your blood in sputum, as I said you probably might have allergic rhinopharyngitis, and blood is coming from the nose or throat.
You may be allergic to food allergens like banana and curd. Also look out for other aeroallergen, like dust, pollen, fungus. These are the aeroallergen which commonly cause respiratory allergies in nose, throat and lung. Allergy skin prick test may help your doctors to pick up other allergic substances.

If you are positive for any one of them, then immunotherapy works well.
If you are not allergic to any aeroallergen, then romilast with steroid nasal spray for
3 months would help.

Regarding yellow urine indicates highly acidic urine. You can take urine alkanizer and plenty of liquids
Please send the ultrasound abdomen report after it is done for me to comment on it.
