Taking anti coagulants like cervinaf and ecosprin is of most importance. Taking these medications will prevent clogging of the arteries.

Detailed Answer:
Hello
Thanks for getting back
This is what I suggest
1. Medical line of management is preferred in grade 2 piles.
2. Constipation should never happen in patients who have undergone cardiac surgery as straining might hamper coronary vessel flow and cause another cardaic event
2. I suggest you to try medical line of management without surgery for 10 days. if the bleeding stops and constipation improves then please re start anticoagulants.
3. If the bleeding re starts after starting anti coagulants then please get her operated for the piles. My worry is if the anti coagulants are stopped for too long she might end up having a cardiac event
4. Approach a gastroenterologist for injection of sclerosing agent into the piles, it is easy, fast and less invasive. It uses lesser anesthetia with a faster recovery than traditional surgery.
I hope I was of help, if you have any further queries please get back to me
