Brief Answer:
Painless hematuria

Detailed Answer:
Hello and welcome,

I assume you meant to write blood in the urine?

Blood in the urine (hematuria) can be from many causes, some innocuous, some more serious. So it is important to follow up with his doctor.

Painless (I am assuming his urination is painless) hematuria in men can be from:

Strenuous exercise - it is not known entirely why this happens, but it is an innocuous cause for hematuria.

Anticoagulant medications and meds that increase likelihood of bleeding - aspirin, heparin, coumadin, ibuprofen and other NSAIDs

Urinary tract stones - sometimes these are "silent"/not painful

Benign prostatic hypertrophy (enlarged prostate)

Kidney inflammation and injury - autoimmune disorders such as vasculitis can do this; kidney damage do to diabetes or a physical blow to the kidney such as in a car accident or fall; environmental toxins; radiation

Inherited diseases - sickle cell, Alport syndrome, etc.

Cancer of the kidneys, bladder, or prostate

So it is important now to follow up. The next step is an ultrasound exam of your kidneys, or your doctor may want to get a CT or MRI. But imaging is the next thing.

I hope this information helps. Please let me know if I can provide further information.
