What Causes Breathing Difficulty While Sleeping?

Question: Sir/Madam,
My question is regarding a peculiar health condition faced by my dad, he is 67 yrs old, he is facing problem of severe breathing trouble and chest pain while sleeping, he is not able to histrave any food after 4 in the noon, if he takes food, the breathing trouble and chest pain becomes severe, he has frequent swelling in his legs.
He has a medical history of nervous stroke 15 yrs back, had a minor heart attack before 6 yrs, and has had problem of internal fistula 8 years back, for which he was operated, but continues to have it, he is very fragile now and weak, please advise the treatment that can be given to him.

cardiac cause must be rule out first.

Detailed Answer:
Hello dear, thanks for your question on HCM.
Your father is at risk of cardiac disease. The risk factors in his case are
1. Old Age
2. Past history of heart attack.
3. Past history of brain stroke.
And present symptoms like breathlessness on sleeping (orthopneoa), chest pain with breathing difficulties even with minimal exertion like eating and swollen legs, all are favouring congestive cardiac failure (CCF).
So we need to rule out this first.
So consult cardiologist and get done ECG and 2D Echo to rule out CCF.
Your father will need admission in hospital for treatment. It will be difficult to administer cardiac drugs at home. He may need diuretics, inhaled bronchodilators, inotropic drugs and other cardiac supportive drugs.

Hello Doctor,

Your detailed answer was indeed of great help, I have got my dad admitted in hospital and they have confirmed a block in his arteries and thrombosis in his heart, they have found a swelling in his splene, and also informed about reduced level of sodium in his blood.

The breathing trouble has subsided, but doctors have informed that his blood platelets count is less around 2 lakhs and the stage is cancerous, medics have ruled any surgical process and are now administering an oral drug warfarin, mainly.

My dad still is very weak,doesn't have hunger and is barely able to walk, kindly advise what further steps can be taken to improve his current health condition.

1. Have you undergone coronary angiography?
2.What is site of thrombosis? In heart vessels or peripheral vessels? Please upload scan copy or image of report and other blood reports on right side of your dashboard.
3. Have you rechecked his sodium now?
