i read your question and i understand your concern. I believe the bug is H pylori for which you have been put on antibiotic combination therapy. That can cause ulcers and explains all this back and shoulder pain symptoms.

vagus nerve alone never causes such symptoms and is always in correlation with a complete neurological correlation. Dont complicate things yoru yourself. Its mere gastritis and needs supportive care and antacids for management.

i can understand the concern. These antibiotics may be a discomfort some times but its a necessary evil to get rid of H pylori. You may take some supportive care like antacids and motility enhancing medication and diet lifestyle modifications. Talk to your doctor and see if they have any chance to reduce the dose. There is therapy with 5 meds for 10 days.
