What Causes Brown Vaginal Discharge After Nexplanon Implantation?
Question: I am a 23 year old female who has been on the Nexplanon contraceptive for 4 years (on my second implant). I have had the same sexual partner for 3 years. I have been experiencing brown vaginal discharge for about a month which I know can be a common side effect of this contraceptive. However, the discharge is now accompanied with lower bilateral quadrant abdominal pain upon palpation (sharp direct pain), lower back pain, right sided flank pain, yellow urine no matter how much I drink, frequent headaches , and new onset of vivid dreams. I have a strong family history on both sides of uteran and ovarian cancer. I also had chlamydia when I was 19 that was treated promptly. What would you recommend?

Hi,
I have gone through your medical history carefully and understood your concerns.
It is true that brown discharge is often associated with Nexplanon contraceptive. However, it is also true that pelvic inflammatory disease together with genital infection are also common side effects of using Nexplanon.
To my opinion, if I was your caring doctor, would advise to:
- consult your OG specialist as soon as possible
- get testing of the vaginal discharge (microscopic examination and culture)
- get a retesting for chlamydia too
- run an abdominal ultrasound
For the moment, I advise to continue drinking plenty of fluids, keep good genital hygiene, avoid sexual intercourse until betterment and take anti-inflammatory medications (e.g ibuprofen or mephenamic acid).
As you have been with the same sexual partner for the last 2 years, I advise to get him tested too.
