What Causes Burning Sensation During Urination?
Question: Hi, I am 24 years old and since a year I am having a burining sensation while urinating and masturbating. Also, at times when I masturbate I feel pain near the opening of my meatus. I have undergone a six months treatment and the doctor said its protate infection and cant be cured completly. Earlies I had pus cells in my urine. However, I had a urine test two weeks ago and it was normal. Another doctor said that drink water and stop masturbating. I am helpless. Kindly suggest.

Pus cells in urine indicate an infectious cause. However, this should have been followed by urine culture and sensitivity. Most likely the prostatitis was diagnosed based on ultrasound findings. Your doctor is right in saying that chronic prostatitis is a difficult condition to treat and doesn't resolve completely even with months of antibiotic use. However, urinary tract pathology including concretions or stone at the neck of bladder can lead to such symptoms. X ray KUB can be done to look for the possibility. Urine alkalinization with potassium citrate can be done accordingly to facilitate the removal of a small stone. Irritable bladder syndrome is another cause though with IBS, there is increased frequency and nocturia which is more predominant. Cystometric studies can be advised if the urine test is normal and no anatomical cause is found. Excessive and rough masturbation can sometime injure the urethral meatus. Till the symptoms resolve , try to abstain from any vigorous sexual activity. If there is any associated urethral discharge then it should be sampled and sent for routine examination and C/S testing. If you have had unprotected sex prior to the onset of these symptoms then STD panel can also be done to be on the safe side.

Hi, Thank you for you response.
I dont think I have stone as I have undergone an ultrasound. Also, I am attaching the reports of the test I undergone when the doctor diagnosed that it is Prostrate infection. I had six months course of all the required medicines. And now the pus cells is not there as i had a test two weeks ago. So even if the infection isnt their I am having the same symptoms. I am not sexually active and I always have used protection. The area around my meatus is a bit black and the XXXXXXX area is pink in color. May be it indicates anything. Also my penis goes towards left. when it is flacid it hangs toward left and when erected it goes toward left. So may be there is a physical damage? If required I can send you the images of my penis. And as mentioned by you if there is a damage to urethra then how can it be treated? As I am young and soon will be maried. So if I will have pain after ejacuation then it will affect my sexual life as well. As even when I masturbate then the whole act is fine. however, at the the time of ejacuation I feel pain just when its about to discharge. The pain is near the opening. I cant explain exactly. Please help!

The blackened meatus as you mentioned needs to be examined. If there is any local inflammation or infection then local treatment with topical combination of antibiotics , anti fungal and steroids can be advised.
The left shift of the penis , when erect is not significant and does not indicate physical damage.
The symptoms you have described indicate an infection of the urogenital tract. Seminal fluid examination can also be done.
The urethra needs to be examined. A retrograde urethrogram and cystoscopy would be the last option if empiric therapy fails and urethral damage is suspected e.g due to a stricture or infection and may require surgical intervention to address the cause. Azithromycin or ceftriaxone can be prescribed for the possible urethritis. You need to obtain a specialist urologist opinion in this regard.

The Blackening of the XXXXXXX skin around meatus is said to be pigmentation by my dermatologist as well as urologist. I dont know what it is. Thougj it appears ugly. Coming on the major concern, I am afraid if it is a physical damage and if it is then can it be treated completly. Also, is it going to affect my sexual life.? The test u mentioned are done by injecrting fibre in the urethra. So is it completly safe to undergo those tests? As mentioned by you regarding the test for seminal fluid. Can you tell that what exaclty is it for? is there any possiblility of an infection in my semen?
Also, in the meanwhile should I restrict masturbating? I generaly do it once a day and at times I skip a day in between. I feel urge to do masturbate. My urologist said that its prostate infection. So I believ I should consult again with some other urologist regarding my case.
