Question: My 84 year old mother has been complaining for a few days about hotness on back of her head, today is the first day it is enough to keep her from getting out of bed. A cold towel on the back of her head made her feel a bit better, but she is saying that basically, her whole head is uncomfortable.

I appreciate your concern

The scalp needs to be examined for any localized lesion, redness or swelling. Lymph nodes associated with the area should also be examined for any swelling or tenderness. If the local examination is normal then neurological and systemic causes need to be looked for. Blood pressure should be checked and visual acuity should also be assessed. If there is any focal neurological deficit then brain imaging should be advised. Basic metabolic panel, ESR, Complete blood count should be done as part of the workup. Gabapentin and tramadol can be advised as treatment to look for improvement, empirically if no cause is determined.
