What Causes Burning Sensation In The Tongue In A Diabetic With Acid Reflux?

Question: I have a constant burning on the root of my tunge and on the roof of my mouth. my throat is sore. I have no cold symtoms and the burning is brought on by eating. I do suffer from Acid reflux. and have been trated for years with proton pump inhibitors first prylosec, then Nexium 2, 40mg 1 every 12 hours. now I am on dexilant 1 60 MG 2wice per day. Nothing is working to moderate the burn and my doctors are just kinda throwing up their hand and saying I am a strange case. I hurt so bad I don't know what to do and I am losing hope. I am A Type one Diabetic on Isulin, I take lantus, metformin, arenenol, valsartan. along with the dexalant an a zantac at bed time. I had an egd test done 2 weeks ago and biopsies were take for barrits and came back negative. please can some one help I feel like I am going to die.

There is a rare condition called ZES (Zollinger Ellison Syndrome) which results in severe acidity that does not respond well to standard ant-acid treatment.

Moreover, the metformin that you are on can cause stomach upset in various ways. Discuss with your doctor if you can try staying off it for a week or so and see if the symptoms improve.
When I see someone like you in my practice, I typically order the following blood tests in addition to a detailed physical examination:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
HbA1c (Glycosylated Hemoglobin = your last 3 months' glucose average). Also known by other names such as GlycoHemoglobin or Glycated Hemoglobin or A1c
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH (checks your thyroid)
Free T4 (this too checks your thyroid) Thyroid autoantibodies : Anti Thyroid Peroxidase (TPO) antibodies and Anti Thyroglobulin antibodies 12 hour Fasting Lipid profile
Urine albumin to Creatinine Ratio (early sign of diabetes affecting the kidney)
25 hydroxy Vitamin D levels (ideal range 40 to 60 ng/ml = 100 to 150 nmol/liter)
