Brief Answer:
Cardiac chest pain most likely.

Detailed Answer:
Welcome to Healthcaremagic.
Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
Your history is classically suggestive of Cardiac chest pain that is Angina.
Breathlessness and pressure over the chest and throat represents angina pain in your case.
However, Acid reflux can also be a possibility, but is least likely as per your symptoms.
Cardiac chest pain is severe and crushing in nature. It increases on activity like walking, exercising. It often radiates to left arm, shoulder and nape of neck.

So, first of all you should get an ECG and Chest X-ray done.
If the ECG shows any signs of cardiac issues, next line of investigations would be an ECHO and Treadmill Test (TMT). If any abnormality is found, then an angiography should be performed.
You should also get your Fasting blood sugar, Lipid profile, and Thyroid profile done.
This is my personal opinion based on details available here. If you still have any other query, you may please ask me.
Stay Healthy.

Brief Answer:
Yes, it is

Detailed Answer:
Hello, Yes, definitely increase in pulse rate is also an indicator of angina as the heart is trying to pump blood more and more, and it is unable to do that efficiently, so this becomes the reason for increase in pulse rate.
You should see your doctor now.
Good Luck.
