To recapitulate: Male/30 - H/O hunger feeling pains for 15 years which used to wake you up from from sleeping - also have indigestion and frequent diarrhea - Abdominal pain for more than a year - which comes and goes for weeks or months - mostly right upper but now all over abdomen and sometimes to breast bone and back - wants suggestions...
My suggestions after reading the history you have provided are:
I would advise the following investigations and treatment in a such a case:
Investigations:
*Routine blood investigations like Complete blood picture, blood sugar, urea, creatinine, liver function tests, thyroid function tests.
*Urine -routine and microscopy
*Stool: routine, microscopy, occult blood, culture and sensitivity.
*Ultrasonography of abdomen,
*Colonoscopy
*Upper GI Endoscopy...
* A good clinical evaluation by a General Surgeon or a Gastroenterologist..
All these things can help us to get the exact diagnosis.
One an exact diagnosis is done further treatment is easy....

Since you have a long standing problem, please get all the tests done.
This is very important protocol to be followed to get a proper diagnosis. Then only we can come to a proper diagnosis.
